The 2026 DSH/DY15 UC Application is now available to request online. It is important for each hospital to request their facility’s DSH/UC application via the online link below from HHSC prior to September 25, 2025. If you do not submit a DSH/UC application request, you will not receive the DSH/UC tool from HHSC and therefore would be unable to participate in the program.
Please complete the following steps:
- Request your facility’s DSH/UC application via the online link below from HHSC.
- As you fill out the application request, please enter Discovery’s information as the Primary Application Contact so we can track and coordinate all data needs.

- Please note: If your facility would like to add a secondary application contact name, please enter the information for one of the contacts below.
- After completing the form, please keep a print screen to confirm the application request was successfully sent to HHSC and email a copy of this to dshuc@daacg.com.
As we get closer to the Sept 25th due date, we will be contacting your facility to confirm that you have submitted the application request and later that you have received the application itself.
