  • Who is required to complete the survey? Any LGE that submitted IGT between October 1, 2021 and September 30, 2022 for any of the following programs: DPP-BHS, ICF-UPL, TIPPS, RAPPS, DSRIP (for non-hospital services), QIPP, UC-Dental, UC-Physician, and/or NAIP. 
    • Completion of this survey is required by 1 TAC §355.8704. HHSC will not accept future IGTs from LGEs that fail to complete the survey before the deadline. 
  • Where do I send supporting documents?  
    • Submit supporting documents by email to PFD_LFM@hhs.texas.gov. Include your LGE Name in the Subject of the email and Document Names. 
    • NEW! – if you have any large files, please request access to our secure FTP site by emailing PFD_LFM@hhs.texas.gov.  
  • Didn’t I just report on FFY22 in October? Some LGEs transfer for both hospital and non-hospital services. This survey will cover the same time period (FFY22; October 1, 2021 – September 30, 2022), but will only collect information on non-hospital programs.
